Apprize Online Submission

Instructions

1. Complete the Apprize pdf file by typing your entries directly in to the pdf file.
2. Save the pdf file on your computer.
3. Complete the submission form below with your name and email address..
4. Use the “Attach files” section to upload files:
• File 1 must be the completed Apprize form in pdf format. This field is required.
• Files 2 and 3  should be jpg files
• File 4 may be used to submit a Featured Article (in Microsoft Word format).
5. Press the Submit button one time to send the files.
